About Your Appointment

On your first visit, we will begin a permanent record*. For your protection, we must know the condition of your health before treatment is started. Be prepared to provide a detailed medical history including your present medications. If the history form has been enclosed in your information packet, complete it and bring it to your appointment. And, please bring your eyeglasses, regardless of their condition, and ALL of your insurance cards to your appointment. If you are unable to keep your appointment, or are going to be late, please call the office as soon as possible.

We have an extensive library of printed and video infor-mation. If you are interested in any of the information for yourself, your school, club, or civic organization, we would be happy to make it available.

We appreciate all comments and suggestions regarding our services. Please tell us so that we may continue to enhance our level of service to you, your friends, and family.

* Your medical records are strictly private. The only exception is when required by law, as in industrial injuries, assault, etc.

Office Staff and Appointments
On your first visit, we will begin a permanent record. For your protection, we must know the condition of your health before treatment is started. We will ask you to complete a brief medical history form. If the history form has been enclosed in your information packet, fill it out and bring it with you. Be prepared to provide a detailed medical history including your present medications. And, please bring your eyeglasses, regardless of their condition, and ALL of your insurance cards to your appointment.

If you are unable to keep your appointment, or are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to other patients.

We work very hard to stay on schedule. However, emergencies sometimes arise. If we are delayed for a prolonged time, we will do our best to notify you beforehand. Please assist up by being on time gor your appointment. Occasionally, we may take patients out of turn if they are urgent. We will ask your patience if you have to wait. We know your time is valuable.

Please allow enough time for your appointment. Complete exams may take an hour or two. For certain examinations, we use dilating eye drops which may blur your near vision for approximately two hours and make your eyes sensitive to light. plan your visit accordingly. If you do not have sunglasses, we will be happy to provide you with a temporary pair, free of charge. Treatment or follow-up examinations require 30-60 minutes.

If you have an emergency, we will do our best to respond to your problem promptly. Please call our office as early in the day as possible. If the problem arises when the office is closed, our answering machine will give you an emergency number for Dr. Stoken or one of his associates. The doctor will return your call as soon as possible. If they are not available and immediate attention is necessary, it is best to go to the Emergency Department of the nearest hospital where the physician on duty will begin treatment.

Charges for office visits and surgery are determined by the time spent and vary with the severity or complexity of the problem. A general list of fees for all services provided is available upon request.

Our office participates in a variety of insurance plans. Please bring all insurance cards with you.

We encourage our patients to call with any questions concerning treatment or exam results. However, as a courtesy to patients undergoing treatment, the doctors will be unable to answer every telephone call. Our technicians are specially trained to answer your questions concerning prescription refills, etc. If you need to speak to a doctor personally, the technician will take your information, and the doctor will return your call when conditions permit.

Many diseases have similar symptoms. If you have developed a complication or a new problem, please understand the need for re-examination in the office.

If you have been referred to us by another doctor, please tell us so we may report back promptly. Your medical records are strictly private. The only exception is when required by law, as in industrial injuries, assault, etc.
